Trucking business registration - what you need:

 

1) Personal information

• name

• address

• SS number

• FEIN number

• phone number

 

2) Truck's proof of purchase

• title or signed front & back

• bill of sale (we can make one for $40)

 

3) Four (4) recent bills (within 45 days) to verify provided address in Illinois:

• phone bill required

• gas

• cell phone

• electric

• cable

• property tax

• rent agreement

 

4) CDL copy

 

5) If a corporation: articles of incorporation (must be Illinois's)

 

6) If truck is leased: lease agreement (we can make one for $50)

 

7) If date of purchase is prior to 60 days: 2290 - Highway Tax

 

8) Information about the company for which the truck will be driving or will be leased
